24.5c Measure/Record Urinary Output

Supplies: graduate paper towel toilet sink bedpan with liquid in it recording device

Must successfully demonstrate all 8 steps

Don clean gloves before handling bedpan

Pour contents of bedpan into graduate without spilling/splashing “urine” outside of container

Rinse the bedpan and pour rinse into toilet

Measure the urine amount at eye level with container on a flat surface  If the measurement is between two lines, round up to the nearest 25mL

After measuring urine, empty contents of graduate into toilet

Rinse graduate and pour rinse into toilet

Properly remove gloves and wash hands before recording

Required Step: Record urine output within 25mL  of evaluator’s reading

Suggested Time: 4 minutes

Must successfully demonstrate 12/15 steps

Explain procedure, speaking clearly, slowly, and directly, maintaining a face-to-face interaction

Provide 360° privacy with a  curtain, screen, or door.

Before providing mouth care, ensure client is in an upright sitting position (75-90°)

Don clean gloves  before cleaning the client’s mouth

Place cloth towel across chest  before providing mouth care

Secure cup of water (hot & cold) and moisten toothbrush

Before cleaning mouth,  apply toothpaste to moistened toothbrush

Required Step: Clean the client’s mouth  (including tongue and all tooth surfaces) using gentle motions

Maintain clean technique with placement of toothbrush

Candidate holds emesis basin to client’s chin while client rinses mouth

Wipe candidate’s mouth and  remove the clothing protector

Dispose of used linen  in the soiled linen container

Rinse the toothbrush and  empty, rinse and dry the emesis basin, leaving mouth care supplies by the sink

Properly remove and dispose of gloves and immediately wash hands

Ensure signaling device is within reach and the bed is in the lowest horizontal position

5c Provide Foot Care for One Foot

Supplies: 4 washcloths 1 towel bath basin soap lotion Soiled linen container

Must successfully demonstrate 14/18 steps

Explain procedure, speaking clearly, slowly, and directly, maintaining a face-to-face interaction

Provide 360° privacy with a  curtain, screen, or door.

Before washing, check water temperature (self and client) for safety and comfort

Place basin in a comfortable position for the client on a protective barrier

Don clean gloves before washing client’s foot

Place client’s bare foot into the water

Apply soap to wet washcloth

Lift foot from water and wash entire foot, including between all toes

Rinse entire foot,  including between all toes

Dry entire foot with dry washcloth,  including between all toes

Apply lotion to top and bottom of foot (excluding between toes) and remove excess lotion with a dry washcloth

Repeat steps 3-11 for other foot (for state testing only wash one foot)

Support client’s foot and ankle throughout the procedure

Empty, rinse, and dry the basin

Place the basin in the designated dirty supply area

Dispose of used linen  into soiled linen container

Properly remove and dispose of gloves and immediately wash hands

Ensure signaling device is within reach
