22c Measure/Record Weight

Supplies: non-slip shoes scale recording device

Must successfully demonstrate all 7 steps

Explain procedure, speaking clearly, slowly, and directly, maintaining a face-to-face interaction

Ensure client is wearing non-skid shoes before walking to scale (You can ask client to put on shoes)

Before client steps onto scale, set the scale to zero, ensuring the pointer is in the middle

Ask the client to step to the center of the scale and obtain the client’s weight

While client steps off scale, stand next to the scale and assist client, if needed, off the scale

Wash hands before recording

Required Step: Record the weight within 2 lbs (or 0.9 kg) of evaluator’s reading Must record either lb or kg after number

Suggested Time: 4 minutes
