33c Transfer Bed  to Wheelchair  Using a Transfer Belt

Supplies: wheelchair transfer belt non-skid shoes

Must successfully demonstrate 16/19 steps

Explain procedure, speaking clearly, slowly, and directly, maintaining a face-to-face interaction

Provide 360° privacy with a  curtain, screen, or door.

Before assisting to wheelchair, ensure bed wheels are locked  (You may need to move the bed first to allow enough room for the wheelchair transfer)

Before assisting to stand, ensure bed is in the lowest horizontal position

Before assisting to stand, fold the wheelchair footrests out of the way  (Recommend doing this before placing  w/c beside the bed)

Before assisting to stand, position wheelchair alongside bed, at the head of bed facing the foot of the bed or at the foot of bed  facing the head of the bed

Required Step: Before assisting to stand, lock both wheels on the wheelchair

Required Step: Before assisting to stand, assist client to sit on the side of the bed with feet flat on the floor

Before assisting to stand, place client’s  shoes on his/her/their feet

Before assisting to stand, place transfer belt on the client securely at the waist over clothing

Before assisting to stand, provide instructions to enable the client to assist in transferring, including a pre-arranged signal to alert the client when to begin standing

Stand facing the client to ensure the safety of the candidate and client during the transfer  Count to 3 to alert the client to begin standing

On signal, gradually assist the client to stand by grasping the transfer belt on both sides with an underhand grasp while maintaining stability of the client’s legs by standing knee-to-knee or toe-to-toe with the client

Assist client to turn to stand in front of the wheelchair and back up until he/she/they can feel the wheelchair against his/her/their legs

Lower the client into the wheelchair

Position client with hips touching the back of the wheelchair and remove the transfer belt

Position the client’s feet on the footrests

Place signaling device within reach

After completing skill, wash hands

Instruct client to inform you if pain is experienced during the exercise  (Recommend placing client in supine position)

Required Step: While supporting the arm at the elbow and wrist, raise client’s straight arm from side position upward toward head to ear level and return arm to side of body (flexion/extension) at least 3 times unless pain is verbalized  Move joint gently, slowly, and smoothly through the ROM, discontinuing exercise if client verbalizes pain

Required Step: While supporting the arm at elbow and wrist, raise client’s straight arm away from the side of the body to shoulder level and return to side of body (abduction/adduction) at least 3 times unless pain is verbalized  Move joint gently, slowly, and smoothly through the ROM, discontinuing exercise if client verbalizes pain

Place signaling device within reach and ensure bed is in lowest horizontal position

After completing skill, wash hands

Required Step: While supporting the leg at the knee and ankle, bend the knee and then return the leg to the client’s normal position (flexion/extension) at least 3 times unless pain is verbalized Move joint gently, slowly, and smoothly through the ROM, discontinuing exercise if client verbalizes pain

Required Step: While supporting the foot and ankle close to the bed, push/pull foot toward head (dorsiflexion) and push/pull foot down (plantar flexion) at least 3 times unless pain is verbalized Move joint gently, slowly, and smoothly through the ROM, discontinuing exercise if client verbalizes pain

Place signaling device within reach and ensure bed is in lowest horizontal position

After completing skill, wash hands

Supplies: meal tray hand wipes chair

Must successfully demonstrate 12/15 steps

Explain procedure, speaking clearly, slowly, and directly, maintaining a face-to-face interaction

Before feeding, look at name card on tray and ask client to state his/her/their name

Required Step: Before feeding client,  place client in upright (75-90°) position

Place the tray where the client  can easily see the food

Clean client’s hands before beginning feeding

Sit in a chair facing client during feeding

Tell client what foods and beverage  are on the tray

Ask client what he/she/they  would like to eat first

Using a spoon, offer client one bite of each type of food on the tray, telling the client the content of each spoonful

Offer a beverage at least once during the meal

Ask client if they are ready for the next bite of food or sip of beverage

At the end of the meal,  clean client’s mouth and hands

Remove the food tray

Leave client in an upright (75-90°) with signaling device within client’s reach

After completing skill, wash hands
